Supports in this model are available 24/7 unless identified in the ISP. The Family Living provider is responsible for the day to day implementation of services. The qualifications for the Family Service Provider are the same as direct support staff: approval of the provider is contingent on State approval. Family services are monitored minimally by one face to face contact each monthly with both the provider and person served. The independent contractor/provider of services reports directly to the Service Coordinator. Training, knowledge of emergency procedures, etc. are required of the provider, documented and monitored by the Service Coordinator.
CIHS are intermittent services and/or supports that are individually designed to instruct or enhance home living and community skills and to address health and safety as needed. CIHS provides people the opportunity to design and manage the services and/or supports needed to live their own home or their family home.
CCS are based upon the preference and choices of each person and designed to measure progress towards Desired Outcomes specified in the ISP. Activities include adaptive skill development, adult educational supports, citizenship skills, communication, social skills, self-advocacy, informed choice, community integration and relationship building.
Respite is a flexible family support service. The primary purpose of respite is to provide support to the person and give the primary-unpaid caregiver time away from duties.
